Purpose:
Workers in Ontario use WSIB Form 6 to report a work-related injury, illness, or disease to the Workplace Safety and Insurance Board (WSIB). - Healthcare Details: Information about healthcare providers consulted (e.g., doctor, hospital, clinic).
- Work Status: Whether the worker missed time from work, returned to work, or is on modified duties.
Worker’s Responsibilities:
- Complete and submit Form 6 after the injury or illness as soon as possible.
- Keep a copy for personal records.
- Notify the employer of the injury or illness (if not already done).
