WSIB Form 6: Worker’s Report of Injury/Disease

Purpose:

Workers in Ontario use WSIB Form 6 to report a work-related injury, illness, or disease to the Workplace Safety and Insurance Board (WSIB). The form is a critical part of the claims process, ensuring the worker’s account of the incident is documented. It also initiates or supports a claim for workplace compensation benefits.

Purposes:

  1. Notify WSIB of an Injury or Illness: Provides details of the workplace incident or condition leading to the injury or illness.
  2. Document the Worker’s Perspective: Captures the worker’s version of events, symptoms, and medical treatments.
  3. Trigger Compensation Benefits: This helps determine the worker’s eligibility for benefits such as loss of earnings, healthcare coverage, or rehabilitation support.

Key Information Collected:

  1. Worker’s Details: Name, address, date of birth, and contact information.
  2. Employer Details: Employer’s name, address, and contact information.
  3. Injury/Illness Details: Description of the incident, symptoms, and date/time it occurred.
  4. Healthcare Details: Information about healthcare providers consulted (e.g., doctor, hospital, clinic).
  5. Work Status: Whether the worker missed time from work, returned to work, or is on modified duties.

Worker’s Responsibilities:

  • Complete and submit Form 6 after the injury or illness as soon as possible.
  • Keep a copy for personal records.
  • Notify the employer of the injury or illness (if not already done).
  • Provide honest and complete information to avoid delays or issues with the claim.
