WSIB Form 7: Employer’s Report of Injury/Disease

Purpose:

Employers in Ontario use WSIB Form 7 to report a workplace injury, illness, or disease to the Workplace Safety and Insurance Board (WSIB). It ensures the employer’s account of the incident is documented and initiates the worker’s compensation benefits claim process.

Key Objectives:

  1. Notify WSIB: Provides details about the workplace incident or condition that caused the injury or illness.
  2. Document the Employer’s Perspective: Records the employer’s version of events and actions taken following the incident.
  3. Compliance with Reporting Requirements: Ensures the employer meets their legal obligation to report workplace injuries and illnesses within three (3) business days of being notified.

Key Information Collected:

  1. Employer Information: Details about the employer, including company name, address, and contact information.
  2. Worker Information: Worker’s name, position, and employment details (e.g., start date and regular duties).
  3. Incident Details: Date, time, and description of the incident, including factors that contributed to the injury or illness.
  4. Medical Information: Details about medical treatment provided and healthcare professionals involved.
  5. Work Status: Whether the worker continued working, returned to work, or required time off due to the injury.
  6. Steps Taken by the Employer: Measures implemented to prevent a recurrence of the incident.

Employer Responsibilities:

  • Complete and submit Form 7 to WSIB within 3 business days of being notified of the incident.
  • Provide a copy of the completed form to the worker.
  • Support the worker in accessing medical treatment and returning to work.

Importance:

Filing Form 7 promptly ensures:

  • The worker’s claim is processed without delay.
  • The employer’s account is included in the investigation of the claim.
  • Compliance with WSIB regulations to avoid penalties.
  • Notice to the Joint Health & Safety Committee (if relevant)
